New Patient Intake Forms
Please download, print, and fill out the New Patient Packet and either mail the forms to our office or fax them to 310-279-5019 (on the cover sheet, please write: Attention: Dr. Forghani). These forms should be sent to Dr. Forghani at least 2 days prior to your first office visit. Please note that appointment length will not be extended due to late arrival. We are happy to mail, fax, or email patient forms to you if you are unable to download them from our website. You may fill out the forms completely at your convenience and bring them with you to your first appointment. Otherwise, kindly arrive 30 minutes prior to your scheduled appointment so that you have adequate time to fill out the forms in our office.
